How much each no-show costs your clinic, in real dollars
Most multidisciplinary clinic owners have never calculated this number. It usually hurts more than it looks.

The number you've never calculated
If I asked you right now how much it costs, in hard cash, every time a patient no-shows without warning, you probably wouldn't know how to answer. Most multidisciplinary clinic owners I've diagnosed didn't know either.
And it's a simple calculation: the therapist's idle hour, the space's fixed cost, electricity, materials, and the reception time already spent organizing the schedule. Add it all up, multiply by the number of no-shows in the month, and the result is usually a shock.
Why this number stays invisible
That happens because a no-show doesn't show up as an expense in any accounting report. It's an opportunity cost, money that never came in, not an invoice that went out. And what doesn't show up in the report, the owner doesn't see, and what he doesn't see, he doesn't fix.
The ripple effect on the team
There's a second effect, even quieter: the therapist left idle by a no-show loses rhythm, loses productivity in the next session, and the pedagogical coordinator has to scramble to reorganize the schedule. The real cost is higher than the simple math of an empty slot.
What to do with this number
In the strategic diagnostic I run, this is one of the first numbers I calculate with the clinic owner. Once you see the annual figure, the decision about confirmation policy, waiting lists, and no-show fees stops being a question of "being too strict" and becomes a question of math.
If you've never done this math in your clinic, that's the first symptom that your management is handling operations, but hasn't reached the finances yet.
